If you have done, are currently doing, or may do business with the City of Whiteville, please provide the requested information as listed below and submit to the City of Whiteville. These forms can be submitted via online or they can be mailed to the City of Whiteville, PO Box 607, Whiteville, NC 28472.
1. Please complete and submit the attached W-9 form.
2. We will also need you to contact your insurance provider and request they forward to us a copy of your certificate of liability insurance. It can be mailed directly from your insurance provider to the City of Whiteville, at the above address, or it can be emailed to vbellamy@ci.whiteville.nc.us
If workers compensation coverage is NOT included on the certificate, please complete the Employee vs. Contractor form and submit to the City of Whiteville.
3. In addition, as a result of House Bill 786 which became law on September 4th, 2013, the City of Whiteville is requesting your company complete the E-Verify Affidavit to the City of Whiteville for compliance purposes.
The new law prohibits local governments from contracting with contractors/companies who do not comply with E-Verify requirements and the law also applies to contractor’s subcontractors. Please understand your company is not obligated to provide the form W-9, certificate of insurance and the E-verify affidavit; however, failure to do so could inhibit the City of Whiteville from contracting with your company.
